Job Description of a Practice Nurse Jobs in Reading:

We are seeking an experienced practice nurse to join our healthcare team in Reading. In this role, you will also work collaboratively with other healthcare professionals to provide high-quality care to our patients. You will be responsible for delivering a range of services, including chronic disease management, health promotion, and immunizations.

• Provision of health education and medical advice to patients
• Management of long-term conditions and monitoring of patient’s health status
• Will be the first point of contact for patients with minor injuries and illnesses.

1. Active NMC registration
2. Experience working as a Practice Nurse
3. Excellent communication and interpersonal skills
4. Ability to work collaboratively with other healthcare professionals
5. Experience working in a primary care setting preferred

Qualifications of a Practice Nurse:

• Bachelor’s degree in nursing or equivalent
• Additional qualifications in chronic disease management and/or health promotion preferred

Experience of a Practice Nurse:

A minimum of 2 years experience as a Practice Nurse
Experience in a primary care setting preferred
